ok, when I opened my new op amp package from from Mammoth Electronics I dropped the chip on my workbench beside the original op amp. I thought I picked up the new one when I started testing but I guess I grabbed the original by mistake (they are identical).
Just by chance I switched the op amp chip tonight to see if I would get a clean signal with the effect engaged. To my surprise, I got a chorused signal and the LED turned on when I clicked the switch. It even works with my proper AC adapter.
The final solution was changing the burnt out op amp chip and the surge protection diode. All is working once again. Thanks to everyone who helped me on this journey.
